• Care information you choose to share, such as descriptions of the daily challenges, needs, or circumstances of the person who may receive care.
• Employment and credential information from individuals who apply to work with us, including background screening, reference, and qualification information required for caregivers in Tennessee.
• Service records created while providing care, such as care documentation, scheduling records, and communications with our office.
• Limited technical information that our website host and form provider automatically record in order to deliver and secure this website.

Some of this information is sensitive and may include personal, health, employment, or identification details. We collect it only when it is needed for agency operations, caregiver screening, care coordination, eligibility verification, billing, compliance, or another purpose described in this policy.

• Verify eligibility and coordinate with TennCare CHOICES managed care plans and other authorized payers
